Please complete this form to let us know what activities your student is interested in participating in this fall. This form is NOT a commitment to any of the listed activities; it is just an interest form. Once you submit this form, you will be contacted by the coach/advisor this summer with additional signup information.

Middle School Fall Activities Interest Form

The Board of Education shall provide equal and bias-free access for all students to all school facilities, courses, programs and services, regardless of race, creed, color, national origin, ancestry, age, marital status, affectional or sexual orientation, gender, gender identity or expression, religion, disability or socioeconomic status.
